Our advanced practice nurse practitioners (APNs) ensure a smooth transition from hospital to home. Our APNs work closely with you to adjust medications, provide education, and improve the outcomes for patients experiencing advanced heart failure. The team can also administer intravenous diuretics in an outpatient setting to keep you at home and out of the hospital.
